Unsuccessful applications and reviews

Why your application may have been unsuccessful

My application has been declined? What can I do?

Not all people with disability, including those with support needs, are eligible for a Companion Card. Only people with severe or profound disability who require lifelong substantial attendant care to attend venues or events, in accordance with the scheme, are eligible for a Companion card.

If you don’t meet the eligibility criteria your application will be declined.

If your application has been declined, you can seek a review if:

  • your circumstances change and/or
  • you’re able to provide new information that clearly demonstrates you meet the eligibility criteria of the Companion Card.

It is not necessary to complete a new application form.

You will need to:

  • notify the Companion Card program of your request for a review in writing
  • provide additional information, which may include letters, reports, assessments, etc.
  • have the health professional sign any new information and provide their contact details for verification (please note original documents cannot be returned to you).

Any new information you provide, along with the information obtained in your original application, will be considered as part of your review. This review may take up to 12 weeks and you will be notified of the outcome of the review in writing.
