You will need to provide your Victorian address in the application form.

If you are not an Australian citizen but are a permanent resident, you will also need to prove your residency by providing your visa category. 

Applicants who live outside of Victoria need to contact their own state's Companion Card office. 

The Companion Card is not issued to every person with disability. There are some eligibility indicators for the card that provide some guidance. 

High level attendant care support includes significant assistance with mobility, communication, self-care or learning and where the use of aids, equipment or alternative strategies does not enable you to participate. Companion Cards are not issued to applicants where the support needs are intermittent (just in case) or who need reassurance and/or encouragement.

Reports or evidence must be provided as to the level of functional impairment.

The card can only be issued if you can show you have a lifelong need for high-level attendant care.

The card is not issued for conditions with infrequent or unexpected events such as allergic reactions, falls or medical emergencies. 

Companion Cards cannot be issued if you may become more independent in the future from treatment/management, training, recovery or developmental improvements.
