You’ll need to fill out the industry affiliation form if you’d like your business or organisation to affiliate with the companion card program. You’ll also need to read the Industry Handbook.
Information to help you complete the form:
- You’ll be asked (Q.3B) to describe the range of services provided and/or to list specific venues/activities. It’s recommended that any high-profile services or venues that are part of your organisation are specifically listed so we can then add them to the affiliate search for cardholders.
- If your organisation has multiple outlets or locations that trade under the same name (eg. a chain of cinemas), you don’t need to list every location on the form.
If you have any questions about participating in the program, email your query to companioncard@dffh.vic.gov.au.
Alternatively, you can telephone the Victorian Companion Card Information Line on 1800 650 611.
